Islamic Emirate of Afghanistan (second Taliban rule)
Who & why: as US and NATO forces complete their withdrawal under the February 2020 Doha Agreement, the Taliban take Kabul on 15 August 2021 while the internationally backed government collapses and President Ashraf Ghani leaves the country. The Taliban re-declare the Islamic Emirate of Afghanistan, restoring the same movement and name that governed from 1996 to 2001.
What it represents: a white flag with the black shahada (Islamic declaration of faith), which Wikipedia says was reinstated on 15 August 2021 after the Taliban victory. It is the design of the 1997-2001 period in outline, but Flags of the World notes that photographs from August 2021 show a larger shahada and, on some flags, an added inscription below, and Wikipedia shows a variant with 'Islamic Emirate of Afghanistan' in Pashto; the claim that it was readopted unchanged is not supported. The tricolor of the 2004-2021 republic is no longer the flag of the state in Afghanistan.
Wikipedia (Flag of Afghanistan): the Islamic Emirate flag, a white field with the black shahada, was reinstated on 15 August 2021 after the Taliban victory; a variant carries 'Islamic Emirate of Afghanistan' in Pashto beneath. FOTW: the Taliban flag flew at the Kunduz border crossing on 8 August 2021 with the shahada much larger than the 1996-2001 flag and an extra inscription below; its historical index lists 'Since August 2021' as a separate period.

Not established: Neither source says the 2021 flag is identical to the 1997-2001 one: FOTW notes a larger shahada and added inscription, so 'readopted unchanged' is not supported. Wikipedia's adoption dates are 27 October 1997 and 15 August 2021; FOTW's own date for the current flag (4 January 2004) is the tricolour's. The 15 August 2021 date is Wikipedia's; FOTW shows the flag in use from 8 August.
