People's Democratic Republic of Algeria
Who & why: the Algerian War of Independence (1954–1962), fought by the National Liberation Front (FLN) against French rule, is one of the 20th century's most significant and costly decolonization conflicts — marked by guerrilla warfare, the Battle of Algiers, and widespread civilian suffering on both sides before the March 1962 Évian Accords and a nearly unanimous July 1962 independence referendum end 132 years of French control.
What it represents: the green-and-white flag with a red crescent and five-pointed star, used by the FLN and Algerian nationalists since the 1930s–40s, becomes the flag of the new republic at independence and remains unchanged since.
Both sources give 1962, the year this page starts the current flag. Wikipedia's infobox gives the adoption as 3 July 1962; Flags of the World dates it 3 July 1962.

Not established: Only the year the current flag was adopted was compared; the design and the swatch colours were not checked against a specification.
