Malaysia (federation)
Who & why: on 16 September 1963, the Federation of Malaya merges with Singapore, Sabah, and Sarawak to form Malaysia. Singapore's own uneasy fit — both economic tension and communal politics between its majority-Chinese population and the peninsula's Malay-majority leadership — leads to its separation, by mutual (if strained) agreement, on 9 August 1965, after which the remaining thirteen states plus federal territories continue as Malaysia.
What it represents: fourteen alternating red and white stripes and a fourteen-point star, originally for the federation's fourteen states and territories at founding — the design was kept unchanged after Singapore's departure, now read as representing the states plus the federal government.
Both sources give 1963, the year this page starts the current flag. Wikipedia's infobox gives the adoption as 26 May 1950; Flags of the World dates it September 1963.

Not established: Only the year the current flag was adopted was compared; the design and the swatch colours were not checked against a specification.
