Islamic Republic of Mauritania (current flag)
Who & why: a constitutional referendum on 5 August 2017, under President Mohamed Ould Abdel Aziz, approves a package of amendments that includes adding a red stripe along the top and bottom edges of the national flag; the new design is formally adopted later that month. The government's stated rationale ties the red to the blood shed by Mauritanians in the struggle for independence and in defense of the homeland; the change is also widely read by commentators, inside and outside the country, against the backdrop of Mauritania's long and unresolved reckoning with slavery, though that reading was not the government's official framing.
What it represents: the same green field, gold crescent, and gold star flown since 1959, now bordered top and bottom by narrow red stripes — the only change made to the flag since independence.
Both sources give 2017, the year this page starts the current flag. Wikipedia's infobox gives the adoption as 25 October 2017; Flags of the World dates it 1361.

Not established: Only the year the current flag was adopted was compared; the design and the swatch colours were not checked against a specification.
